Once your payment is confirmed, you’ll receive a consent form and an “About You” form, usually within 24 hours. Please complete both to finalise your booking and help you get the most out of your consultation.
There’s no need to download or email anything — both forms are updated online automatically.
If you have a complex medical history or feel you have a lot to discuss, please consider booking a double appointment (two consecutive time slots). We will combine these into a single one-hour consultation.
